- Abstract : Objective: Radiofrequency catheter ablation (RFCA) is one of the main strategies for atrial fibrillation (AF) treatment, although the outcome is less effective for persistent AF (PeAF). The aim of this study was to identify the predictor of successful catheter ablation of PeAF. Design and Method: A total of 135 consecutive patients (mean age 62.1 ± 10.5 years, male 100 [74.1%]) from March 2011 to February 2015 were enrolled. Patients who underwent successful cardioversion and RFCA for atrial fibrillation were included. After conversion to sinus rhythm, maximal and minimal P wave duration, P wave dispersion and P wave voltage was measured using Prucka Cardio Lab System. P wave dispersion was defined as the difference between the maximal and minimal P wave duration among the different ECG leads. Results: Forty (29.6%) patients showed recurrence of atrial tachyarrhythmia during follow up. P wave dispersion was significantly longer in recurrence group (41.3 ± 22.3 vs 32.8 ± 16.8, p = 0.03) and other parameters showed no statistical significance. According to logistic regression, longer P wave dispersion was associated with recurrence after RFCA (HR 2.6, 95% confidence interval 1.1–6.1). Conclusions: After cardioversion, measurement of P wave characteristics, especially P wave dispersion, can be a predictor of successful catheter ablation in patients with persistent AF using simple method.
