- Abstract : Objective: To evaluate the circadian oscillations of the arterial wall stiffness in hypertensive patients depending on their stage of the disease. Design and method: The study included 30 hypertensive patients aged between 35 to 65; 24 males and 6 females. They were divided into 2 groups depending on the stage of the disease and organ damage. The first group included 15 patients with stage I and stage II hypertension and the second included 15 with stage III. With the help of BP monitoring BPLab company Petr Telegin (Nizhny Novgorod) for each patient we calculated the average value of their; systolic blood pressure, diastolic blood pressure, the propagation time of the reflected wave, arterial stiffness index, augmentation index, and also the time of day variability of these parameters. Results: In patients with stage I-II hypertension (group 1) there is less variability in their the propagation time of the reflected wave than in patients with stage III hypertension (group 2): 18 ± 5, 1 vs 24, 8 ± 10, 7 (p = 0, 018) respectively. And group 1 patients had significantly lower SBP compared with the second: 8, 4 ± 124.9 vs 134.4 ± 10, 6 (p = 0.005), respectively. Conclusions: In patients with more severe hypertension with complications such as myocardial infarction, stroke, or in combination with diabetes there is a large diurnal variation of the reflected wave propagation time. Which makes this a sign of a negative evaluation of the ABPM data.
