<abstract> <title> <x xml:space="preserve">Abstract</x> </title> <sec> <title>Background and Importance:</title> <p>Gliosarcoma (GS) is a rare variant of glioblastoma multiforme containing distinct gliomatous and sarcomatous components.</p> </sec> <sec> <title>Clinical Presentation:</title> <p>We report a rare case of secondary gliosarcoma arising from an anaplastic astrocytoma. The patient was a 42-year-old man who was diagnosed with a GS in the right temporal lobe at a second operation after diagnosis of brain tumor 10 months ago. Initially, he underwent a neuronavigation-assisted resection of the "brain tumor, " as the magnetic resonance spectroscopy showed right temporal and intrinsic insular mass that was suspected as glioma. Histologic examination showed anaplastic astrocytoma (World Health Organization grade III). Then he received radiotherapy and chemotherapy. Ten months after the first operation, magnetic resonance imaging revealed that the brain tumor had recurred. Then the second operation was performed. The tissue obtained from partial removal of the tumor revealed GS. The history, neuroimaging, and histopathology of this patient are discussed.</p> </sec> <sec> <title>Conclusions:</title> <p>Our case will be helpful for the diagnosis and treatment of GS.</p> </sec> </abstract>
