<abstract abstract-type="author-highlights" id="abs0005"> <title id="sect0005">Highlights</title> <sec> <p id="spar0005"> <list id="lis0005"> <list-item id="lsti0005"> <label></label> <p id="par0005">All known safety problems with national scale IT systems in England over 6-years were examined.</p> </list-item> <list-item id="lsti0010"> <label></label> <p id="par0010">National IT implementation was associated with problems on a large scale impacting on care delivery.</p> </list-item> <list-item id="lsti0015"> <label></label> <p id="par0015">Problems encountered are not unique, but are well-understood challenges of IT implementation.</p> </list-item> <list-item id="lsti0020"> <label></label> <p id="par0020">Human factors were four times as likely to result in reported patient harm than technical problems.</p> </list-item> </list> </p> </sec> </abstract>
