- <abstract abstract-type="main" xml:lang="en"> <title> <x xml:space="preserve">Abstract</x> </title> <p> <bold>BACKGROUND:</bold> In the Rh blood group system, variant RhD and RhCE express several partial antigens. We investigated <italic>RH</italic> in samples with partial DIVa that demonstrated weak and variable reactivity with anti‐C.</p> <p> <bold>STUDY DESIGN AND METHODS:</bold> Standard hemagglutination techniques, polymerase chain reaction–based assays, and <italic>RH</italic> sequencing were used.</p> <p> <bold>RESULTS:</bold> DNA analysis showed that six red blood cell (RBC) samples with weak and inconsistent reactivity with anti‐C lacked <italic>RHCE</italic>*<italic>C</italic>, but all had <italic>RHD</italic>*<italic>DIVa</italic>, which encodes partial D and Go<sup>a</sup>. We then tested RBCs from 19 Go(a+) cryopreserved samples (confirmed to have <italic>RHD</italic>*<italic>DIVa</italic>) with four anti‐C and observed weak variable reactions. <italic>RHCE</italic> genotyping found all but one of the samples with <italic>RHD</italic>*<italic>DIVa</italic> also had <italic>RHCE</italic> nt 48G&gt;C and 1025C&gt;T, named <italic>RHCE</italic>*<italic>ceTI</italic>. Alloanti‐c was found in a patient with c+ RBCs and <italic>RHCE</italic>*<italic>ceTI</italic> in trans to <italic>RHCE</italic>*<italic>Ce</italic>, and alloanti‐e was found in a patient with e+ RBC and <italic>RHCE</italic>*<italic>ceTI</italic> in trans to <italic>RHCE</italic>*<italic>cE</italic>. <italic>RHD</italic>*<italic>DIVa</italic> in trans to <italic>RHD</italic> erroneously tested as <italic>RHD</italic> hemizygous.</p> <p> <bold>CONCLUSIONS:</bold> <italic>RHD</italic>*<italic>DIVa</italic> and <italic>RHCE</italic>*<italic>ceTI</italic> almost always, but not invariably, travel together. This haplotype is found in people of African ancestry and the RBCs can demonstrate aberrant reactivity with anti‐C. <italic>RHCE</italic>*<italic>ceTI</italic> encodes partial c and e antigens. We confirm that <italic>RHD</italic> zygosity assays are unreliable in samples with <italic>RHD</italic>*<italic>DIVa</italic>.</p> </abstract> … (more)
