Abstract

<abstract abstract-type="main" id="bjh12817-abs-0001"> <title>Summary</title> <p>The influence of genetic variation on warfarin dose requirement is limited for paediatric patients. We performed a retrospective, cross‐sectional study to examine the effect of variant <italic>CYP2C9</italic> and <italic>VKORC1</italic> genotypes on warfarin dose in 100 children. Those with <italic>VKORC1</italic> genotype AA required 48% of the dose of homozygous wild‐type (GG, <italic> P </italic>&lt; 0·0001). Patients with any variant <italic>CYP2C9</italic> allele required 71% of the dose for wild‐type (<italic>P</italic> = 0·001). The effect of variant <italic>VKORC1</italic> alleles tended to vary with age, suggesting developmental ontogeny may influence warfarin sensitivity. Age, <italic>CYP2C9</italic> genotype, <italic>VKORC1</italic> genotype and age:<italic>VKORC1</italic> interaction accounted for 53% of warfarin dose variability.</p> </abstract>
